* Therapy Dog Handler Course Workshop Agreement (print hardcopy)
This is an Agreement between Dog Training for Owners and the Handler named above, wherein Handler has enrolled in a Pet Partners Therapy Dog Handler Course Workshop hosted by Dog Training for Owners. The date and time of the Course Workshop was selected by Handler on the Dog Training for Owners website. The fee for the Workshop is $119. Payment shall be made in advance and is non-refundable.

Therapy Dog Handler Course
The Course will be taught by an Authorized Pet Partners Instructor, and covers policies, procedures, and skills necessary to safely visit with Handler's dog in environments such as hospitals, nursing homes, classrooms, libraries, mental institutions, correctional facilities, and more. The course will be held over and 8-hour period. Completion of the Course does not guarantee that Handler's dog is suitable for therapy work; a separate evaluation is required to provide such an assessment.

Health and Safety
Dog Training for Owners will do everything it reasonably can to conduct the Course in a manner that is safe for the Handler, any animals that may be present, and other people. While many factors are beyond the control of Dog Training for Owners, the below terms and conditions are aimed to help ensure a healthy and safe environment, within reason.

I acknowledge that Therapy Dogs may be present at the Course and agree that the risks involved include, but are not limited to, myself contracting a communicable disease or being bitten.

Responsibility and Liability
I acknowledge and agree that I am solely responsible for my own dog and my own dog’s actions in the future. I acknowledge and agree that any instruction or information I receive during the Course does not constitute any guarantee whatsoever that my dog will not cause harm or loss, intentional or unintentional, to a human being, animal or property. I hereby hold Pet Partners, Pet Partners Instructors and Dog Training for Owners harmless of any liability for my dog or any of my dog’s actions including, but not limited to, intentional or unintentional actions that cause harm or loss to a human being, animal, or property at any time in the future.

I hereby hold Pet Partners, Pet Partners Instructors and Dog Training for Owners harmless of any liability for any loss or damage to me, my dog or my property including, but not limited to, loss or damage from disease, other persons, other animals, my dog running away, fire, or other unavoidable causes.

This Agreement shall be deemed to have been made in, and shall be construed pursuant to, the laws of the State of California. This Agreement shall be binding on the heirs, administrators, personal representatives, and assigns of the Handler, Pet Partners, Pet Partners Instructors and Dog Training for Owners. Any controversy or claim arising out of or relating to this Agreement, or the breach thereof, or as the result of any claim or controversy involving the alleged negligence by any party to this contract, shall be settled in accordance with the rules of the American Arbitration Association, and judgment upon the award rendered by an arbitrator may be entered in any Court having jurisdiction thereof. The arbitrator shall, as part of his/her award, determine an award to the prevailing party of the costs of such arbitration and reasonable attorney's fees of the prevailing party.

* If your dog is on a prescription medication, aside from heartworm and flea/tick preventative, we recommend you contact Pet Partners directly to ensure that the prescription medication is acceptable in therapy work, before signing up for the Workshop.

** Therapy dogs must have proof of Rabies vaccine. Rabies Titer is not acceptable.

*** Therapy dogs cannot be on raw diets.
